Manage forms
View guest and service forms
On MyZen, tap Appointments.
To view the guest's profile, tap the appointment, tap the tile with the guest's name, and go to Forms.
Here you can view all the completed and incomplete guest and service forms.
To access guest form history, next to Guest Form, tap History. Here you can filter and view all the forms.
Fill a guest form
On MyZen, tap Appointments.
To view guest's details, tap the appointment, tap the tile with the guest's name, and go to Forms.
You can also access forms directly from the appointment details. The orange icon indicates an incomplete form and green icon indicates that the form is completed.
Guest forms filled and not filled will appear under Guest Form.
Service forms filled and not filled will appear under each service.
Tap the form that needs to be filled and enter the required details.
If it is a group of forms, fill the form and tap Submit & Fill Next.
If it is an individual form, fill the form and tap Submit.
If you do not want to submit and only save the form, tap Save & Close.
