Enable memberships forms and notifications
When guests sign up for new recurring memberships, upgrade/downgrade their memberships, or freeze, or cancel their memberships, you can make the submission of forms mandatory and send the filled-in forms to guests by email.
Required roles: Any role with access to the Administrator mode
Required permissions: None
Enable forms and notifications for membership sales
At the organization level, click the Configurations icon.
To send guests a confirmation about the membership sale and notify them about the upcoming setup fee payment, search for the Send membership sale confirmation notification for setup fee invoice closure for a future sale setting in the Memberships section.
Enable this setting.
To make the submission of a form mandatory in POS for the sign-up of a recurring membership, enable the Sign-up of a recurring membership - Mandate form submission in POS (Web and ZMA v2) setting.
To send the filled-in form in PDF format by email, enable the Send filled forms in PDF format (email only) setting.
Click Save.
Enable forms and notifications for membership upgrade/downgrade
At the organization level, click the Configurations icon.
To make the submission of a form mandatory in POS for the upgrade/downgrade of a recurring membership, search for the Upgrade/downgrade of a recurring membership - Mandate form submission in POS (Web and ZMA v2) setting in the Memberships section.
Enable this setting.
To send the filled-in form in PDF format by email, enable the Send filled forms in PDF format (email only) setting.
Click Save.
Enable forms and notifications for membership freeze
At the organization level, click the Configurations icon.
To make the submission of a form mandatory in POS for the freeze of a recurring membership, search for the Freeze of a recurring membership - Mandate form submission in POS (Web and ZMA v2) setting in the Memberships section.
Enable this setting.
To send the filled-in form in PDF format by email, enable the Send filled forms in PDF format (email only) setting.
Click Save.
Enable forms and notifications for membership cancellation
At the organization level, click the Configurations icon.
To make the submission of a form mandatory in POS for the cancellation of a recurring membership, search for the Cancellation of a recurring membership - Mandate form submission in POS (Web and ZMA v2) setting in the Memberships section.
Enable this setting.
To send the filled-in form in PDF format by email, enable the Send filled forms in PDF format (email only) setting.
Click Save.
Enable forms and notifications for membership transfer
At the organization level, click the Configurations icon.
To make the submission of a form mandatory in POS for the transfer of a recurring membership, search for the Transfer of a recurring membership - Mandate form submission in POS (Web and ZMA v2) setting in the Memberships section.
Enable this setting.
To send the filled-in form in PDF format by email, enable the Send filled forms in PDF format (email only) setting.
Click Save.
Enable forms and notifications for addition of add-on member
Important
This applies only to businesses with Family memberships enabled.
At the organization level, click the Configurations icon.
To make the submission of a form mandatory in POS for the addition of an add-on member, search for the Enable form submission during add-on member - Mandate form submission in POS (Web and ZMA v2) setting in the Memberships section.

Enable this setting.
To send the filled-in form in PDF format by email, enable the Send filled forms in PDF format (email only) setting.
