This report shows the details of service forms and tag forms saved or submitted during a time period.

Note

If a service has multiple forms, you will see multiple entries in the report.

Column Name

Description

Submit status

  • Saved: Form is filled out and saved but not submitted

  • Submitted: Form is filled out and submitted

Review status

  • Reviewed

  • Not Reviewed

    A form can be in this status if it has not been reviewed or if it has not been marked for review in the first place.

Expiry date

Patient information such as COVID results, height, weight, and blood sugar levels change over time. To ensure up-to-date records, forms can be set to expire after a specific period.

When a form expires, users need to fill it out again with the most recent information. Those who already have access to the form will retain their access even after it expires.
