Forms Review report (v2)

At the end of each week, doctors can use this report to review the service and tag forms submitted by guests.
Front-desk staff can use this report to view the forms that are not submitted and send notifications for those forms.
At the organization or center level, click the Reports icon.
On the Reports page, search for and select the Custom Forms Review Report.
If the report has many columns, scroll horizontally to the right or left to view all columns.
To view specific data on the report, select the desired filters. For more information, refer to report features on Zenoti.
Click Refresh.
If you generate the report for over a year, click the Email button.

Guest Code | Unique code that identifies the guest |

Appointment Date | Date on which the service was performed |

Submit Status | Status of the form |
Review Status | Review status of the form A form is in the Not Reviewed state if either it is not marked as reviewed or the form is not eligible for review. |
Last Modified By | Name of the user (guest or employee) who recently performed an action (save, submit, or review) on the form |
Last Modified On | Last date on which a user performed an action on the form |
Enable for Review | Form is enabled for review |
Expiry Date | Date on which the form is set to expire |
Job | Job role of the provider associated with the appointment |
Appointment Status | The current status of the appointment linked with the service |
Review forms with Forms report v2

Click the Reports icon.
Search and select the Forms report.
Alternatively, under Loyalty, select the Forms report.
Select the appropriate filters as in the table below and click Refresh.
Select forms and click Review.
You can select a maximum of 50 forms for review.
To move to the next or the previous form, use the navigation buttons.
You can choose to add your signature on the pop-up and click Review forms.

Impacts and considerations
To save a customized view of the report, after selecting the desired fields, click Save view and enter a name for the view.
If you select more than ten forms for review, selected forms will be loaded in batches of 10.
To complete the review of all the forms in the current batch, click Review and load next batch.
If you add a signature, it will be appended to the reviewed form(s) along with your name, the reviewed date, and the timestamp.
If you choose to review forms without a signature, only your name, the date, and the timestamp are appended to the reviewed form(s).
The signature you add here will be updated on your profile.
Example
Example: You want to view forms that are assigned for the Aesthetic treatment procedure from September to February and look specifically for skin types that are more prone to allergies, with the degree of susceptibility as 7(on a scale of 1 to 10).
