Guest forms report
The Guest Forms report allows you to track and manage all guest forms. It allows you to view, filter, and take action on guest forms. You can view guest forms (V3 and V2) by status, Not Filled, Saved, or Submitted for guests with appointments within the selected date range. The report provides details regarding the guest forms, enabling data analysis based on form status and appointment dates.
This report is enabled for all owners by default. Owners can grant access to other users from Security roles page.
At the organization or center level, click the Reports icon.
On the Reports page search for and select the Guest forms report.
If the report has many columns, scroll horizontally to the right or left to view all columns.
To view specific data on the report, select the desired filters. For more information, refer to report features on Zenoti.
Click Refresh.
If you generate the report for over a year, click the Email button.
The following table describes the filters available in guest forms report and their corresponding descriptions. You can use these filters to generate the report.
Filter | Description |

Guests with Appointments Between | Select a date range to view guest form data. The range is set to Last Week by default. |
Centers | Select the centers to view guest form data. All centers where you have permissions are selected by default. |
Form status | Filter the report based on the form status.

The following table describes the columns you see in the Guest forms report with a description for each column.
Column | Description |

Center | Center where the guest form was submitted. |
Guest Code | Unique identifier for each guest. |
Guest Name | Name of the guest. |
Form Name | Name of the form being tracked. |
Form Status | Current status of the form such as not filled, saved, submitted. |
Last Modified By | User who last modified the form. |
Last Modified On | Date and time when the form was last updated. |
View | Link to view the guest form. |
Send Guest Form Link | Allows the user to send a link to the guest for completing the form. |
The title of the report is determined by the configuration for the Guest Label. For example, if the guest label is set to "Patient," the report title will display as Patient Forms instead of Guest Forms.
