Send and Manage Employee Surveys

This article explains how an organization-level manager and administrator can use the Employee Survey feature at the organization level including:

  • Setting up access permissions

  • Creating reusable survey forms

  • Scheduling surveys for targeted recipients

  • Delivering surveys via the MyZen app

  • Tracking responses and survey status

Overview

The Employee Survey feature provides a centralized way to distribute surveys across your organization. Surveys are delivered through the MyZen app, allowing employees and service providers to respond quickly and conveniently.

From Zenoti, managers can:

  • Define the survey audience—organization-wide, center-specific, or individual employees

  • Schedule delivery and set expiry timelines

  • Monitor progress through survey status indicators (Scheduled, Live, Completed)

  • Review and analyze responses directly within the platform

Key terms
  • Survey Form: A reusable form built using the form builder, linked to a survey.

  • Scheduled: Survey is planned for future delivery.

  • Live: Survey is currently active and visible to recipients.

  • Completed: Survey has been filled out and submitted by recipients.

  • MyZen: Mobile app used by employees or service providers.

Prerequisites
  • Before creating or managing employee surveys, you must assign the correct security role permissions. Only users with the necessary permissions can access the Employee Survey feature. For more information on how to set survey access, refer to the Security role permissions for employee manager functions article.

  • Ensure that View or Edit is selected for Service Custom Data from the Appointment Book section under Security Roles permissions. For more information on how to grant access, refer to Enable permissions to view and edit forms data.

Enable Employee Survey

  1. From the organization level, navigate to Configuration > Zenoti Labs.

  2. Turn on the Enable Employee Survey toggle.

  3. From the organization level, navigate to Employee > Employee Survey.

This opens the Manage Employee Survey page where you can:

  • View all surveys

  • Check their status

  • Create new forms and surveys

Create a survey form

Before scheduling a survey, you must create a survey form:

  1. Click Create survey form.

  2. On the Create New Form screen, enter the Name, Code, and Description.

  3. Select Employee Survey as the Form Type.

  4. Choose Form Builder from the Create Form Using field.

  5. Click Save and Proceed.

Create and schedule a survey

  1. At the organization level, navigate to Employee > Employee Survey.

  2. Click Create Survey.

  3. On the Create New Employee Survey page, complete the following sections:

    • Enter Survey Name – Provide a name for the survey.

    • Select Survey Form – Choose the required form from the dropdown.

    • Schedule Date / Time – Define when the survey should start.

    • Due Date / Time – Define when the survey should expire.

  4. Choose recipients:

    • All employees of the organization

    • Employees of selected centers

    • Specific employees

    Communication Channel is set to MyZen Push notification by default.

  5. Click Save.

The survey appears in the list with Scheduled status.

View and track survey status

Each survey displays one of the following statuses:

  • Scheduled: The survey is created but not yet delivered.

  • Live: The survey is active and visible to recipients.

  • Completed: The survey has been submitted by recipients.

You can view and track surveys by navigating to Employee > Employee Survey.

Survey completion via MyZen

  • Once a survey goes live, recipients receive a notification on the MyZen app.

  • They can open, complete, and submit the survey within the app.

  • Upon submission, the survey status updates to Completed.

Considerations
  • Surveys can only be created at the organization level.

  • Survey form must be created before creating the survey.

  • MyZen app is required for recipients to complete the survey.
