Configure guest custom form requirements for class enrollment

Requiring a guest custom form for class sign-in is a way to collect additional student information before the class begins. Fitness studios can choose to make the form mandatory or optional when students enroll in a class or event.

The rules for filling out the guest form apply uniformly to students enrolling from the Webstore, ZMA, or when studio staff enrolls students from the Roster view or CMA.

Required roles: Administrator or Center Managers

Required permissions: None

Make guest form optional for class enrollment

  1. At the center level, click the Configurations icon.

  2. Search for and select Require guest custom form to sign in setting from the Fitness section.

  3. Select the Not required option.

    Students can register for the class without completing the guest form.

  4. Click Save.

Send reminder to submit guest form during class check-in

  1. At the center level, click the Configurations icon.

  2. Search for and select Require guest custom form to sign in setting from the Fitness section.

  3. Select the Warn if form not filled option.

    Students are reminded to fill out the guest form, but they can choose to skip it and proceed with class check-in. The reminder appears as a pop-up notification on the respective screens of ZMA, CMA, Webstore, and the Roster view.

  4. Click Save.

Make guest form mandatory for class enrollment

  1. At the center level, click the Configurations icon.

  2. Search for and select Require guest custom form to sign in setting from the Fitness section.

  3. Select the Block if form not filled option.

    Students must fill out the guest form before they can check in for the class. If they haven't filled out the form, check-in is not allowed from the Roster view. In ZMA, there is an option provided to complete the form and proceed with the class check-in.

  4. Click Save.
