Add a signature field to a form

A signature field is a drawing area in the form that captures the user's signature. It can be used for confirming the user's consent for a procedure or as a confirmation of completion of a service or payment.

To view an example of this component, from sample forms, visit the Aesthetics Treatment form. Under Treatment, there is a field for signature.

  1. From the Tools list, drag and drop the Signature component onto the form.

  2. To customize the component, place the mouse pointer on the component and click Edit.

  3. Under the Basic tab, customize the details listed below:

    • To name the field, in the Label field, enter the label.

    • To display text below the signature field, in the Footer label field, enter the text.

    If you want to customize other settings, refer to the table below.

  4. Click Save.

Important

  • The signature field is used to take a guest's consent before a procedure, after completion, and to review forms by the doctor.

  • While reviewing forms, the doctors or the reviewers can add their signature to multiple forms in a single go. So, it is not mandatory to add a doctor's signature field in this case.

  • You can use external signature pad devices to capture signatures. To do this you must talk to a person at Zenoti support to integrate the device with Zenoti.

Perform these steps

Change the background color of the signature pad

Under the Basic tab, in the Background color field, enter a color code for the background of your signature pad.

Example: For the color Beige, you may use the code rgb(245,245,220) or #f5f5dc

Change the color of the pen for a digital signature

Under the Basic tab, in the Pen color field, enter a color code for your pen.

Example: For the color Dark Blue, you may use the code rgb(0,0,139) or #00008b

Ensure that the guest cannot change the contents of the field

For example, fields for the therapist's signature or doctor's notes can be made read-only in the guest's view.

Under the Basic tab, select the Make field read-only for guest (on CMA, Kiosk, Webstore, Zenoti Mobile guest mode) check box.

To not display the field to the guests on CMA, Kiosk, Webstore, ZMA guest mode

For example, fields reserved for office use can be hidden from the guest.

Under the Basic tab, select the Hide field from guest (on CMA, Kiosk, Webstore, Zenoti Mobile guest mode) check box.

Make the field mandatory

Under the Validation tab, select the Required check box

You can make a field appear or disappear on the form based on the value of another field.
