What does the guest data form notification do?
Sends an email to guests with a link to the guest custom data form/intake form. This can be triggered from appointment context menu, guest profile and when new guests are created in appointment book using 'New Guest' menu.
When is the guest data form notification sent?
Sent when the front-desk staff clicks the 'Send guest forms profile link' from the appointment context menu, or from the guest profile.

You can configure an email template.
