Enable alerts for unsigned membership forms
Zenoti displays an alert on the Visits page when a member checks in without a signed membership form. This alert notifies front desk staff so they can collect the required signature before the member enters the facility.
This is useful when memberships are sold remotely, such as over the phone. In these cases, the member has not completed the membership form at the time of purchase. The alert ensures the form is signed during the first visit.
The alert applies to both primary members and add-on members in family memberships.
Enable the Alert
This alert is enabled by default. To verify or update the configuration:
At the organization or center level, navigate to Configuration > Fitness > Essentials.
Select Customize visits page alerts.
Locate the Membership form not signed alert and verify that the Active toggle is turned on.
Click Save.
When enabled, the front desk operator receives a high-severity message in red on the Visits page with the reminder text “Membership form not signed.”
How it works
When a member checks in on the Visits page, Zenoti verifies whether the membership form is signed.
If the form is not signed, the alert appears on the Visits page for that member.
Front desk staff can then ask the member to complete and sign the form.
The alert clears after the member signs the form.
Mandate form submission in POS
You can configure Zenoti to require form submission when a recurring membership is sold through POS. This ensures the membership form is signed at the time of purchase and prevents unsigned forms from carrying over to the Visits page.
At the organization or center level, navigate to Configuration > Memberships.
Select the Membership forms and notifications configuration.
Under Sign-up of a recurring membership, enable the Mandate form submission in POS (Web and ZMA v2) toggle.
Click Save.
