After you create a custom form and upload it to Zenoti, you can assign the form to the services. When you assign a form to a service, the following takes place: 

  • Guests can fill the form details
  • Providers can enter the SOAP notes

To assign custom form with a service

  1. At the organization level, navigate to Admin > Resources > Services
  2. Click Add to create new service or click an existing service from the list to edit its details. 
  3. Do one of the following:
  • If you are adding a new service: Follow the steps in Create a New Service until you reach the Custom section.
  • If you are editing an existing service: Click the Custom tab.

  4. Complete the fields in this tab as follows:
      In the Do you require a service-level form? drop-down list, select one of the
      following options from the list:

  • Same form for each visit: Select this option if you want your guest to fill the same form every time the guest takes a service.
  • Different forms: Select this option if you want your guest to fill different forms, one for the first visit and another for subsequent visits. For example, let’s say you have two forms, Botox Custom form and Botox Follow Up form. You select Botox Custom form for the first visit and Botox Follow Up form for subsequent visits.     Now, when guest visits for the first time, Zenoti applies the Botox Custom Form   and for subsequent visits, Zenoti applies the Botox Follow Up form.
  • No form: Select if the service does not require any form.

 - If you have selected Same form for each visit option, you can specify the following: 

         a) Select a form: Select the custom form from the drop-down list.    
         b) Allow emailing of form link: Select this checkbox to include the form
             link using the [ServiceDataLink] macro in the Appointment Confirmation and
             Appointment Reminder notifications. This allows your guest to access the form
             from their email.
             Note: If you do not select the checkbox, the form link does not get added to
             the notifications and your guest won’t be able to access the form.

 - If you have selected Different forms option, you can specify the following:

         a) First Visit: Select a custom form that you want your guest to fill when they are
             availing a service (the service that you assign this form to) for the first time.
         b) Subsequent Visits: Select a custom form that you want your guest to fill when
             they are availing the same service in their subsequent visits.
         c) Allow emailing of form link: Select this checkbox to include the form
             link using the [ServiceDataLink] macro in the Appointment Confirmation and
             Appointment Reminder notifications. This allows your guest to access the form
             from their email.
             Note: If you do not select the checkbox, the form link does not get added to
             the notifications and your guest won’t be able to access the form. 

   In addition to associating the service forms, you can assign tags to a service. When
   you assign a tag to a service, the form assigned to the tag gets assigned to the
   service. You can assign upto three tags per service.

   Note: When you assign a tag to a service, the assigned custom form is applicable    
   each time your guest avails the service.

   Select Tag Form 1: Select a tag from the drop-down list to assign the tag with the  
   service. 

   Note: For tags, you first create a custom form for a tag,
   create a tag and assign a custom form to the tag, and then assign the tag to a
   service (follow this article to assign a tag to a service). 

5. Click Save.
    Zenoti assigns the service forms and the custom forms (that are assigned to the
    tags) to the service.
