This report shows the details of forms (service forms and tag forms) saved or submitted on a day or during a time period.

Note: If a service has multiple forms, you will see multiple entries in the report.

Navigation

  1. Navigate to the Reports tab and click Try the New Version.

The Reports Dashboard appears.

3. Search for and open the report.
The Forms report opens in a new tab.

4. Depending on the requirement, select the desired filters.
Note: You can view the report only for all the centers you have access to.

5. Click Refresh.
The report is generated based on the selected filters.

Lets you view the report by service categories.

Appointment Date

Lets you select the date range for which you want to view the report.

Submit Status

  • All
  • Submitted
  • Saved

Lets you view the report by various statuses of forms:

  • Saved - The form is saved but not submitted.
  • Submitted - The form is submitted.

Review Status

  • All
  • Reviewed
  • Not Reviewed

Lets you view the report by various review statuses:

  • Reviewed - The form is marked as reviewed.
  • Not Reviewed - The form is either not marked as reviewed or the form is not eligible for review.

Column Descriptions

Column Name

Description

Center Name

The name of the center in which the service was performed.

Invoice#

The invoice number of the service to which the form is associated.

Note: To view the invoice details, click the invoice number.

Guest Code

The unique identifier of the guest who took the service.

Guest Name

The name of the guest who took the service.

Note: To view the guest profile, click the guest name.

Appointment Date

The date on which the service was performed.

Service Name

The name of the service.

Provider

The name of the provider who performed the service.

Form Name

The name of the form associated with the service.

View

The preview link of the form.

Note: Click this link to view the form.

Submit Status

The status of the form.

  • Saved - The form is saved but not submitted.
  • Submitted - The form is submitted.

Review Status

The review status of the form.

  • Reviewed - The form is marked as reviewed.
  • Not Reviewed - The form is either not marked as reviewed or the form is not eligible for review.

Last Modified By

The name of the user (guest or employee) who has recently performed an action (save, submit, or review) on the form.

Last Modified On

The recent date on which the user (guest or employee) performed an action (save, submit, or review) on the form.

Enable for Review

Indicates if the Enable for review option was enabled when the form was configured, with a Yes or a No.

Expiry Date

The date on which the form is set to expire.
